Effective December 1, 2025 Jackson Oncology Associates will be partnered with MediCopy to complete disability/FMLA forms for our patients. MediCopy is fully HIPPA compliant and adheres to all state and federal regulations concerning form completion.
Cost for this service will be $45 for the first form and $15 for each additional form turned in at the same time. After payment is received, disablity/FMLA forms will be completed and sent to the requestor within three (3) business days.
